Relevant data, including TCMS and subsystem inputs, must be collected to track service failures and calculate MDBF/MDBCF monthly. Reliability predictions for major equipment and subsystems must be based on actual revenue service data. A service failure is any event causing a delay over 3 minutes, train unavailability, or withdrawal from service, while pattern failures involve repeated issues beyond expected failure rates.
